Staff Background Checks

Background checks are required for all staff who are living in a University of Michigan Residence Hall and supervising youth groups 17 years of age and younger. This is for the safety of your staff as well as participants in University of Michigan programs.

Process

Download and print the Background Check Form [PDF]. Complete and sign the form for each staff member who will be residing in a residence hall and supervising youth groups.  Forward the completed forms to the Department of Public Safety and Security at the time staff is hired. University Housing will send you the results of the background checks.

Periodic checks will be conducted to ensure that background checks have been submitted for your staff. Additional or replacement staff hired during the course of the summer program must also have background check forms.

Deadline

All forms should be submitted to the Department of Public Safety and Security two weeks prior to the start date of the program. This ensures that the information is processed before your summer program begins.

You do not have to submit all the forms together. To keep the process moving, forward the completed forms as you hire staff.
