Before Your Summer Program Begins

The following should be considered and addressed prior to your group's arrival.

Forms

The University of Michigan requires four standardized forms to be completed for each youth participant (17 years and younger) attending your program.

  • Parental Consent and Release of Liability Agreement (includes authorization for medical treatment)
  • Health Insurance Information Sheet
  • UM Summer Camp Health Questionnaire
  • Physical Examination Information

Door Access Arrangements

All residence halls are locked 24 hours a day, 7 days a week, 365 days a year.

This means that you will need to make access plans for your group on their arrival day and make provisions for late arrivals. You may want to have a “greeter” at the front door to welcome and direct your group.

Please prepare for both arrival and departure days.

It is your responsibility to make these arrangements. University Housing Conference Coordinators and Department of Public Safety personnel will not be available to monitor the doors. The University Housing Conference Coordinator may be able to help you arrange building access for your “greeter” staff.

Participant Information

Before your group's arrival, we will need the names of:

  • Residential participants enrolled
  • Commuter participants enrolled

We use these figures to designate bed space for residential participants and to determine the number of meals to serve.
