Before Your Camp Begins

Forms

The University of Michigan requires four standardized forms to be completed for each youth participant (17 years and younger) attending your program.

  • Parental Consent and Release of Liability Agreement (includes authorization for medical treatment)
  • Health Insurance Information Sheet
  • UM Summer Camp Health Questionnaire
  • Physical Examination Information

Door Access Arrangements

  • Arrival day move-in
  • Escorted campers returning to the residence hall for lunch or other legitimate reason.
  • Commuter identification for meals and other social events in the residence hall.
  • Parents or other visitor identification and access during camp
  • Departure day move-out

Rules

Rules and Regulations

Before your week of camp, we will contact you for the number of:

  • Residential campers enrolled
  • Commuter campers enrolled
  • Meal cards you are purchasing for commuters

We use these figures to designate bed space for residential campers and to determine the number of meals to serve.
